"Questionaire for completion by RCAF returned Evaders, Escapers, or Prisoners of War"

This questionaire is numerous pages for one airman, there were six of these questionaires in my Uncle's file. There are questions/answers pertaining to the demise of thier aircraft up to the last moments prior to bale-out, may include how individual evaded capture or was captured. Complete Personnel file, doubtful if included in brief Geneaology package.
